spiral on surface help

Dear All,

I am currently trying to apply an adjustable spiral onto a particular surface. I have tried many, many things, but have literally only managed to get it into a tangled mess. Is anyone able to point me in the right direction? It seems that I have missed a crucial step, but I don't know what it is! (I am essentially trying to carve the base surface into a long ribbon)
