Polar coordinates

I am trying to apply the following to a list of points which I have converted to a polar coordinate system.

Value of P         Offset to Radius required

 0 to 3               0

3.001 to 4          1

3.001 to 5          2

The new P, T, R then has to be exported into a new list

It has been a long time since I did any VB coding and a have completely forgotten the required syntax.
