Perforation Definition: Not Working Properly

Hi, I found a definition that I liked and re-created it from this website http://formularch.blogspot.com/ title GH:Perforation.  The problem that I am having with the definition is that circles are still falling off of the surface.  It isn't supposed to happen but it is.  The components of the definition that are supposed to solve this are the five at the very top.  I've attached the definition and a sample Rhino model in hopes that someone knows how to solve this issue.
