pattern to poly-surface

Hello all.

I am needing help on figuring out how to transfer my pattern onto a poly-surface, in a way that keeps continuous pattern among the different surfaces.

Also, after I get to this point how would I go about splitting the surface in order for the circles to cause perforations.
