Please Help! i'm getting this error - 1. Surface with ID {12be3297-a3c3-43b1-a2c6-befe0485a3d1} failed to load

Hello. 

I'm very new to rhino and grasshopper, i'm trying to create a mesh screen based on a sample image run through a script created by Sean Madigan, it was working fine and i had an almost complete iteration of what i wanted to do, when recently when i loaded in the script i get this error. 1. Surface with ID {12be3297-a3c3-43b1-a2c6-befe0485a3d1} failed to load.  and nothing will populate in rhino through grasshopper.  Attached you'll find the script,  and base image that i'm using, I have no idea why it won't work and any help would be greatly appreciated, i'm completely out of my depth here.
